Every draft is written against the active treatment plan goals, and Weft tells you which goal today's session did and didn't touch. A goal with no session evidence in 60 days is flagged before an auditor finds it.
The code has to be earned
Weft proposes a CPT code only when the documentation supports it, and shows the sentence that justifies it. Bill 90847 with no mention of who else was in the room and you hear about it at signature, not at appeal.
Signed means signed
A signed note opens read-only with an addendum path — never silently editable, always timestamped, always attributable. The audit log is the product, not a setting you remember to turn on.
Measurement without busywork
